Registrations

You can register here and remain on the list only if you live in the practice area (please ask reception for a map)or click here  Catchment Map. If you do not live in our catchment area, you register with neighbouring GP practices using What is your postcode? (eastlondonregistergp.nhs.uk)

We will not refuse registration on the grounds of 

  • race
  • gender
  • social class
  • age
  • religion
  • sexual orientation
  • appearance
  • disability or medical condition.

To register you can email a  request stating your address details  or submit the completed GMS1 form to 

nelondonicb.theazadpractice@nhs.net.

If you can provide photo ID and proof of address to speed up the process and correctly match your details- please email these with any account details crossed out. 

We will then send you an auto registration link which automatically registers you within 2 days. If there is a problem - we will try to process your details manually or contact you.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

During busy periods like flu season - there can be a delay of up to 4 weeks for a registration health check appointment. We try our best to accommodate times which are suitable to you. Sometimes when the surgery is exceptionally busy and is unable to meet demands- we may have to ask your to come in at particular time but we will be flexible to your needs too. If you cannot wait - please contact NHS111 for details of another GP practice.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Change of Details

If you change your name and/or address, please inform the practice as soon as possible. Please also ensure that we have your correct  telephone number and, if appropriate, your work telephone number where you can be contacted. If there is anything that you do not understand, or wish to have explained again please do not hesitate to ask. If you change your address to an address outside our catchment areas, you will be asked to register with a GP closer to where you live even if your new address is a temporary address.
