Patient Participation Group

The Patient Participation Group (PPG) is made up of volunteer patients who meet with the practice staff to discuss the work of the practice, provide constructive feedback and identify improvements. It is voluntary and we hope one day it will be run and chaired by patients.

All registered patients are welcome to contribute to the Patient Participation Group.

We would like to know how we can improve our service to you and how you perceive our surgery and staff.

We will ask the members of the PPG some questions from time to time, such as what you think about our opening times or the quality of the care or service you received.

We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

Join Our PPG

For the Patient Participation Group to be effective it needs to have a membership that allows it to reach the broadest cross section of the patient population.

If you are interested in joining the Patient Participation Group, please complete the online form.

Alternatively, download and print the document available here, and return to the practice once completed; or let reception know you would like to join by emailing nelondonicb.theazadpractice@nhs.net, calling 020 8475 8550, or visiting the practice.
